The Family Health area within Hennepin County’s Health & Human Services is seeking a Public Health Nurse (PHN) to work with the Caring Connections 4 Kids (CC4K) program. This position will provide screening and assessments to children from birth to 18 years of age who are involved in open child protection cases. This position will work on an interdisciplinary team of PHN’s, Child Protection Social Workers, Child Services Workers, Community Health Workers, and Case Management Assistants to ensure vulnerable children are receiving services and support to promote overall wellbeing. Public Health Nurses working with CC4K will also participate in program development and project-based work for continuous quality improvement. We are looking for applicants who are compassionate, have strong communication skills, and are committed to health equity.

Location and hours:
This position is hybrid and will be performed both on-site at community and county locations and remote as job duties require. Much of the work will be virtual/remote. Work hours will be Monday through Friday, 8 a.m. - 4:30 p.m. with some flexibility. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:

  • Provide individual PHN assessments for children from birth to 18 years involved in child protection cases within the first 30 days of case opening; screen all children for social-emotional and developmental progress; perform assessment and referral for primary medical, dental, and mental health care; review birth and medical records to identify gaps in necessary services.
  • Communicate with biological parents and foster providers to explain the results of the PHN assessment and recommendations.
  • Document results of the PHN assessment and referrals in the Electronic Case File (ECF) and Social Service Information System (SSIS) databases for communication with Child Protection and the court systems.
  • Perform referral and follow-up for infant and early childhood mental health services based on results of social-emotional screening and perform referral and follow-up for Early Intervention, Early Childhood Screening, and Early Childhood Special Education services provided by local school districts.
  • Work in partnership with Child Protection Social Workers, Child Service Workers, and Case Management Assistants to make referrals based on the findings of the PHN assessments.
  • Collaborate with other Hennepin County professionals working in other child and family service programs to ensure that vulnerable children are receiving services that promote overall well-being.
  • Provide regular consultation to Child Protection professionals regarding children’s complex medical needs and Public Health resources available throughout Hennepin County.
Need to have:
  • Licensure as a Registered Nurse issued by the state of Minnesota.
  • Certification as a Public Health Nurse by the Minnesota Board of Nursing.
  • A bachelor's degree or higher in nursing from an accredited school of nursing and six months or more of full-time equivalent nursing experience in maternal-child health, pediatric (early childhood) health, family or Public Health, or a related area.
  • Access to reliable transportation to travel between Hennepin County work sites.
Nice to have:
  • Experience:
    • Using screening and assessment tools in a telehealth environment or over the phone, such as the Ages and Stages Questionnaire, Pediatric Symptom Checklist, and/or Fetal Alcohol Spectrum Disorder.
    • Working with families and children from differing social, economic, racial, religious, and cultural backgrounds who have been impacted by trauma.
    • Working with child protection systems.
    • Working with case management and assessment both virtually and over the phone.
    • Working with ECF, SSIS, MMIS, and/or other database systems.
  • Ability to:
    • Be flexible, highly organized, and self-directed with strong documentation skills.
    • Interact with clients and staff with tact, discretion, and sensitivity.
    • Demonstrate understanding of cultural differences and the impacts of racism on service effectiveness, organizational effectiveness, and on relationships with staff, partners, and those we serve.
  • Effective interpersonal, organizational, computer, and person-centered interviewing skills.
