We are a facility-based team of medical providers who provide medical services to patients living in long-term care facilities. We strive to give quality care to residents while working together with the facility to carry out the best care possible. While being part of an interdisciplinary care team to provide medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.We are currently seeking a Wound CareNurse Practitioner to provide medical services to patients living in long-term care facilities.
NP WOUND CARE ROLE AND RESPONSIBILITIES:
- Provide Wound Care for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications .
- Order, review, and/or follow-up with treatments and diagnostic testing.
- Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.
- Collaborate with primary care physician.
- Provider will assess and monitor residents on a regular basis.
- Provider will be available to assist staff in assessing wound and pressure ulcer risks.
- Provider will evaluate multiple factors related to wounds: i.e.
- Nutrition, Mobility, Incontinence associated dermatitis, Pressure ulcers, Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common dermatologic conditions
MANAGEMENT
- Measures to decrease the risk of wounds with recommendations to staff.
- Measures to optimize healing via protocols
- Superficial wound debridement
- Dressing selection and management
- Placement of negative pressure wound dressings
- Post myocutaneous flap surgery care
NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
- Current, unrestricted NP license in the state of Ohio.
- Board Certification.
- DEA licensure.
- Clinical background in Wound Care.
- Experience working in a long-term care setting.
- Ability to work with an EMR.
- Ability to manage patients with advanced illness and multiple chronic conditions.
- Ability to work within an interdisciplinary care team to coordinate patient care.
- Wound care experience
