Job description

Duties
The Primary Care Service line serves Veterans with varying degrees of psychiatric disorders and conditions and co-morbidities requiring medical and psychiatric interventions. 50% of this position is Program Faculty providing clinical care to Veterans and supervision to Primary Care NP post graduate residents and 50% program coordination for the PCNP Residency Programs.
Major duties and responsibilities include, but not limited to:
  • Supervises PCNP post graduate residents and students in Primary Care clinic setting.
  • Involved in didactic instruction and mentorship surrounding resident EBP projects.
  • Attends residency faculty meetings and participates in faculty development measures.
  • Coordinates compliance and data mining needs for OAA funded programs.
  • Ensures programs are meeting all standards as set in the OAA curriculum and national accrediting bodies.
  • 50%-time Nurse Residency Program Coordinator: Is responsible for the development, implementation, coordination, and ongoing evaluation of evidence-based nursing education Residency Programs.
  • 50%-time Primary Care Nurse PractitionerResidency Program Faculty: Will provide clinical supervision to residents and/or students, assist with responsibilities for the administration of the Primary Care Nurse PractitionerResidency Program at NCHCS. They must be clinically and academically qualified to provide effective leadership to the program in achieving its mission, goals and expected outcomes
Clinical duties include, but are not limited to:
  • Instruct medical/nursing students, residents, and other learners that rotate through the Primary Care medical clinic.
  • Effectively functions both autonomously as well as a member of an interdisciplinary team.
  • Respects patients' cultural and spiritual values
  • Identifies areas for continuing education and participates in in-service training (including staying current with all required education)
  • Utilizes VHA and community resources as appropriate.
  • Maintain an integrated delivery system through expansion of clinical practices. Maintain operational efficiencies through emphasis on safe practices and national safety. Actively implements Lean principles, supports EBP projects, and promotes a culture of continuous quality improvement.
  • Play a key role in the ongoing systematic evaluation and improvement of the quality and effectiveness of nursing care and services as they affect patient outcomes.
  • Initiate interdisciplinary projects to improve organizational performance.
  • Clinical expert and consultant: reviews quality of care for documentation, phone activity, case reviews, chart reviews, etc.
  • Identifies educational needs of staff and ensures that these needs are addressed.
  • Has an integral role in supporting and facilitating patient experiences
  • Encourages staff in team building.
  • Makes sustained contributions to health care by sharing expertise within and/or outside the medical facility.
  • Engages in implementing an educational plan to meet changing program or service needs for self and others.
  • Participates in nursing, medical center or Health Care System committees promoting task accomplishment through interaction and problem solving with group members and other stakeholders.
  • Provide leadership in identifying and addressing ethical issues that impact veterans and staff including initiating and participating in ethics consultations.
  • Use Inductive Reasoning - the ability to combine pieces of information to form general rules or conclusions.
  • Practice Deductive Reasoning - the ability to apply general rules to specific problems to produce answers that make sense.
  • Collaborate in the establishment of and supports a system for recognition of accomplishments and achievements by employees.
  • Participate in the development of education programs to increase productivity and performance. Forecast new knowledge needs for changing practice environment and population groups, planning, implementing, and evaluating strategies to meet those needs.
  • Assess and provide for own educational needs to ensure competence in current and future functional role responsibility in a dynamic health care environment.

Qualifications
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
    • Education Accreditation Requirement. Nursing education must be from a school of professional nursing accredited by one of the following accrediting organizations at the time the program was completed by the applicant:
    • (a) The Accreditation Commission for Education in Nursing (ACEN), 3343 Peachtree Road NE, Suite 850, Atlanta, GA 30326. The ACEN accredits all levels of nursing programs, including clinical doctorate, masters/post-master's certificate, baccalaureate, associate and diploma nursing education programs. OR
    • (b) The Commission on Collegiate Nursing Education (CCNE), 655 K Street, NW, Suite 7, Washington, DC 20001. The CCNE is an accrediting arm of the American Association of Colleges of Nursing (AACN) and accredits nursing education programs at the bachelor and master's level.
    • Foreign Education. Graduates of foreign schools of professional nursing programs, the Nurse Practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent Nurse Practitioner masters or doctorate level degrees who possess current, full, active and unrestricted registration and certification (see paragraph 2c) will meet the requirement of graduation from an approved school of professional nursing to warrant appointment.
  • BASIC CERTIFICATION:
    • Advance Practice Nurse (Nurse Practitioner) must maintain full and current board certification in an area of clinical specialty from one of the following:
    • a. The American Nurses Credentialing Center (ANCC) OR
    • b. The American Academy of Nurse Practitioners Certification Board (AANPCB); OR
    • c. Other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.

GRADE DETERMINATION:
Advanced Practice Nurse (APN) (Nurse Practitioner) II.
(a) Scope. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
(b) Education and Experience Requirement. Meets basic education requirement for APN (CNS). Nurse II is considered the entry grade level for APN (NPs); no experience is required.
(c) Dimension Requirement. APNs (NPs) at level II are expected to meet the following dimension requirements:
  • Clinical Practice. Delivers evidence-based health care, demonstrating proficiency in the ability to manage a patient panel, population or program.
  • Outcome Focus. Utilizes research and evidence-based practice to evaluate the patient care delivery system within a patient panel or program, presents findings and contributes to changes that enhance the quality and timeliness of veteran/patient care.
  • Leadership. Works effectively as a leader within a team/unit/group and seeks interdisciplinary collaboration as appropriate to enhance patient care.
  • Professional Development. Acquires knowledge and skills to develop expertise in area of practice.
  • Professionalism/Collegiality. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Participates in interdisciplinary teams and processes that positively impact patient outcomes.
  • Advance Practice Nurse (APN) (Nurse Practitioner) III.
    (a) Scope. Executes position responsibilities that demonstrate leadership, experience and creative approaches to management of complex client care.(b) Education and Experience Requirement.
    • Meets basic education requirement for an APN (NP).
    AND
    • One year of specialized APN (NP) experience equivalent to the Nurse II grade level.
    (c) Dimension Requirement. APNs (Nurse Practitioners) at grade III are expected to meet the following dimension requirements:
  • Clinical Practice. Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population or program.
  • Outcome Focus. Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations.
  • Leadership. Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population or program.
  • Professional Development. Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility.
  • Professionalism/Collegiality. Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population or program level. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork.

  • Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.
    Physical Requirements: Involves considerable amount of standing, walking, stooping, bending, reaching, lifting and turning. Requires carrying or lifting of objects weighing up to 25 lbs., but occasionally may require work that involves pushing or lifting in excess of 25 lbs. and going up and down flights of stairs. The position requires visual and auditory acuity at a level to provide safe care. The position requires driving between two clinics (Fairfield and Mare Island). Requires use of hands for actions such as typing, simple grasping, fine manipulation and repetitive actions Requires the ability to frequently respond to multiple demands and priorities. Has potential for exposure to infectious diseases, hazardous waste and chemicals. Ability to distinguish shades of colors. Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds. Emotional and mental stability. Represent department or division at external meetings as directed by Program/Medical Directors. Serve as subject matter expert during internal/external training events as directed. Performs minor administrative functions as directed by Program/Medical Directors. Ability to effectively perform the essential functions of the position and respond appropriately in emergency situations. The incumbent is subject to the provisions set by the Nurse Professional Standards Board in areas of probationary period, promotion and other actions requiring board approval.

    Note: Only education or degrees recognized by the Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ) may be used to qualify for nursing positions. The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration.     Additional information
    Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
