Job Details
Remote Nurse Practitioner or Physician Assistant managing a patient panel of adult and geriatric high risk patients.
What you will be doing:
- Patient Evaluation: Conduct thorough assessments of patients' health, encompassing physical examinations, medical histories, and analysis of diagnostic test outcomes.
- Diagnosis and Therapy: Identify and manage a range of acute and chronic illnesses, injuries, infections, and medical conditions. Formulate tailored treatment strategies and prescribe medications when appropriate. • Preventive Healthcare: Administer preventive care services and promote health maintenance, including vaccinations, health screenings, and advising on lifestyle modifications.
- Health Counseling: Provide comprehensive education to patients and their families regarding health conditions, treatment alternatives, medication usage, and self-care techniques.
- Care Collaboration: Work closely with interdisciplinary healthcare teams, including physicians, nurses, and specialists, to synchronize and oversee patient care plans.
- Telemedicine Services: Deliver seamless virtual healthcare consultations, referrals, and ongoing support via our telehealth platform.
- Medication Management: Supervise and adjust patients' medication regimens, ensuring proper dosages and facilitating prescription renewals as necessary.
Skills you need:
- Active NY NP or PA License
- Experience practicing is required
