Nurse Practitioner or Physician Assistant - Geriatric Primary Care
SCHEDULE:

- $110,000 to $140,000 Annual Salary, based on experience

COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth in their Laredo market.
POSITION DESCRIPTION:
The Nurse Practitioner or Physician Assistant (APP) is responsible for managing a panel of SNF residents, focusing on treating in place in collaboration with a full primary care team. The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients’ goals and needs of care. The APP is supported by the APP Manager / Director, as well as the local Medical Director, both of whom provide regular education and guidance on best practices in patient care.
Specific responsibilities include, but are not limited to:
- assess and plan healthcare that is individualized to the patient
- review and interpret medical, surgical, and social histories
- perform screening procedures for patients
- interpret laboratory and other diagnostic data and recommend treatments or actions
- reconcile medications
- be responsible for prescribing or performing various treatment modalities
- work collaboratively with physicians, care coordinators, facility staff, family members, etc.
- perform direct care management support
- educate care teams on patient needs and early identification of changes in condition
- participate in organizational meetings
The practice uses a mobile-based EHR.
The clinical work is autonomous, with one-on-one patient visits and a high level of independence. But it’s also a team-oriented position with regular meetings, conference calls, etc. with other members of the care team.
There are no nights or weekends involved. There may be some flexible, minimal on-call duties, but this will be discussed collaboratively and on-call volume is low, if required at all.
REQUIREMENTS:
- Texas NP or PA license
- DEA
- 1+ year advanced practice experience
- background in Geriatric Care, primary care, facility-based medicine, etc.
- previous experience in SNFs preferred
