Nurse Practitioner Or Pa Or Physician

We perform independent physical exams (consultative exams) for Indiana disability. Each exam last up to 30 minutes.

$40 per claimant seen; work as independent contractor. Make up to $800/day.

Chief complaint/Brief history.

Review of systems/Vital signs.

Basic physical: Mostly range of motion/strength; description of gait; use of hands; etc.

Occasional spirometry testing (0-3 usually) - additional $20/test (takes about 10-12 minutes).

We DO NOT make the decision on who gets disability - we only provide a report as part of the process.

Full Indiana license required - Professional liability insurance provided free of charge.

Usually 20 slots scheduled; 30 minutes each; 8 AM - 5:30 PM; any day of the week except Sundays.

Work as little or as much as you want - usually 1 exam day scheduled per week.

Also have offices in Bloomington and Evansville.
