Overview:Remote based position but must be willing to attend meetings in person.
The NP Navigator serves as the primary point of contact for patients being referred into the Back Pain/Spine Program. The incumbent is responsible for coordinating the care and services of the identified patient population across the continuum of care. The incumbent promotes effective resources utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers. The APP employs self direction and accountability in adhering to evidenced based care protocols for the specified patient population.
Qualifications:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
- Master’s degree in nursing as NP from nationally accredited postgraduate
program - Minimum of 2 years caring for the surgical spine patient, NP experience highly preferred
- Minimum of 2 year in of clinic experience
- Board Certified as APRN in applicable subspecialty required
- RN required
- Prior EPIC experience preferred
- DEA Registration Number Required
Physical Requirements - Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

Pay Range:$48.68 - $84.76 / hour
