YOUR NURSE PRACTITIONER ROLE, WITH US
Internally, we call this role an "APP" (Advance Practice Provider), as an APP you work alongside an interdisciplinary clinical team including registered nurses, nurse care managers, health coaches, registered dietitians, pharmacists, psychiatrists, and social workers - all within the Evergreen Nephrology family. Our clinical team is all driven and motivated to improve quality outcomes, together. Our team will see and address the whole person, maybe for the first time in their care journey.
You will conduct 5D visits in the patient's home. The visit focuses 5 Dimensions of health (Emotional, Physical, Spiritual, Social and Mental). By observing a patient in their own environment and completing a comprehensive evaluation is one of the ways we are able to see and treat the whole patient and eliminate the barriers that historically have impacted your ability to fully understand the patient's needs.

This position requires an active Nurse Practitioner license in the state you are applying in. Acute Care NPs, Family Care NPs and Nephrology NPs all tend to make great candidates for this role.
