We are seeking a highly skilled and compassionate Family Nurse Practitioner to join our healthcare team. - Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Diagnose and treat common acute and chronic conditions, such as infections, injuries, diabetes, hypertension, and respiratory illnesses
- Develop and implement individualized treatment plans for patients based on their specific needs
- Provide patient education on disease prevention, healthy lifestyle choices, and medication management
- Perform minor procedures such as suturing, wound care, and joint injections
- Collaborate with physicians and other healthcare professionals to coordinate patient care
- Utilize electronic medical records (EMR) systems to document patient encounters accurately
```Experience```
To be successful in this role, you should possess the following qualifications:
- Current certification as a Family Nurse Practitioner (FNP)
- Valid state licensure as an Advanced Practice Registered Nurse (APRN)
- Basic Life Support (BLS) certification
- Proven experience in primary care settings
- Proficiency in conducting patient assessments and developing treatment plans
- Knowledge of various medical systems and procedures
- Ability to perform phlebotomy and interpret laboratory results
- Strong case management skills to coordinate care for complex patients
- Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams
Preferred Experience:
- Experience in tube feeding management
- Knowledge of clinical research methodologies
- Expertise in hospice palliative medicine or dementia care
