Pride Health is hiring a Nurse Practitioner in Winnebago County and Waupaca, WI!
3-month contract role on-site at three Senior Living locations in Winnebago and Waupaca Counties.
Shift: Mon-Fri, 8am-5pm.
Pay: $70/Hour
Job Responsibilities:
- Travel between 3 Senior Living facilities during shift
- Conduct comprehensive, patient-facing assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD-10 conditions.
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
Qualifications:
- Active WI NP license
- Basic Life Support (BLS) Certification
- 1+ year of clinical experience in a long-term care setting
- Proficient computer skills including the ability to document medical information with written and electronic medical records
- Understanding of geriatrics, chronic illness, and advanced illness
