Job Description
The Manager, Case Management coordinates the integration of Utilization Management and discharge/transition planning functions to provide effective outcome based, patient-centered care.
Serves as the central resource to the Director of Care Management to develop tools, processes and systems to optimize the department and organizational strategic goals.
This position is eligible for a sign-on and relocation bonus of up to $21,000!
This is a full time (1.0 FTE), exempt position. Work Schedule is Weekdays, Monday - Friday.
Registered Nurse - State of New Mexico or Compact State Nursing License required.
BLS certification within 90 days of hire.
Bachelors Degree in Nursing required; Master's Degree in Nursing is preferred.
Certification in an approved Case Management Program preferred or obtained within one year of hire date.
Five years of relevant acute care clinical experience required.
Relevant personnel management/leadership experience in Mother/Baby required.
Three years of Case Management or Utilization Management experience, in an acute hospital or relevant setting.
