Salary: $85,000 / year
Sign-on Bonus:$4,000; $2000 upon 30 days, $2000 upon completion of 6 months
As a Nurse Liaison, you will function as a nurse educator and consultant in collaboration with other clinicians regarding health promotion and maintenance, medication administration, and acute and chronic medical / psychiatric crisis management for persons served. Nurse liasons will provide support and carry a case load of individuals living within DDS group homes/supported appartments. This includes attending team meetings, monthly reviews, rounding at the various programs, training of employees in various nursing tasks/delegations and participating in a team process of referral reviewed/admissions.

Requirements:- A bachelor's degree in Social Work, Psychology or related field is preferred, but experience may substitute for education. High school diploma or GED required.
- Current R.N. license in state of service.
- 3-5 years experience in nursing.
- Strong time management and writing skills.
- All applicants must be at least 21 years of age and have a current, valid, US drivers' license for at least one year.
- All applicants must become CPR within 2 weeks of employment.
- All candidates must be able to successfully pass a CORI, reference, multi-state background and driving record check.
