Position: Nurse Case Manager II
Pay: $44.14/HR
Hours: Monday - Friday 8am-5pm
Location: A.) Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph, & Van Buren counties and B). Wayne and Macomb County areas.
Position Purpose: The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes
Experience/Education (MUST be included on resume):
- 3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.
- 1 year Healthcare and/or managed care industry experience.
- RN with current unrestricted state licensure
