Description
The Nurse Care Coordinator will work under the general supervision of the Clinical Supervisor. The Nurse Care Coordinator manages the physical health needs and medication management/reconciliation for the Integrated Health Home Program. As the Nurse for IHH, you will also be required to track appointments and any hospitalizations that occur for our clients, ensuring we follow up with them within 24 hours of post discharge. Other duties include but are not limited to, referral scheduling, tracking follow-up results from referrals, understanding health insurance coverage, transition with care, wellness education including sending the member educational materials regarding this and documenting it in their EHR, and health support or lifestyle modification that pertains or is relevant to the client.
As the Nurse Care Coordinator, you will also be responsible for ensuring adherence to the program policies, procedures, COA (Council on Accreditation) standards, licensure and/or contract requirements, and treatment philosophy.

Requirements
- Must be an RN (Registered Nurse) or BSN with an active Iowa License.
- Must provide documentation of education & licensure. College degrees must be documented with original transcripts sent to the Human Resources Director.
- Must have competency in computer basics which includes turning on a laptop, using email, Word, Excel, electronic health record (EHR) systems, provider's portals, etc.
- Must have a valid driver's license to drive for agency purposes.
- Must provide evidence of personal car insurance at time of hire and as renewed, if driving personal vehicle for agency purposes.
- Must have ability to effectively deal with emergency situations.
