- Monitor and ensure the health and safety of all individuals living in the home.
- Administer medications as prescribed by a medical professional.
- Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
- Transport individual to appointments, visits, and activities.
- Implement habilitation objectives/goals as agreed upon by the ISP team.
- When applicable implement behavior building plans.
- Actively interact with individuals in the home throughout your shift.
- Complete all required documentation before leaving shift.
- Assist with preparing monthly activity calendar and weekly menus.
- Ensure the home is clean and in good repair.
- Duties as assigned.
The F&C Home Care Services Provides care for developmentally disabled adults throughout the state of Arizona, in safe and comfortable, family-oriented living situations in residential neighborhoods.
Ratios for staff to member/ client are at minimum 1 staff supporting 1 member.
