Presbyterian is hiring a Physical Therapist to provide unique opportunities for troubleshooting of enviromental obstacles to health and safety. We are seeking a "People Person" who enjoys 1:1 time with patients working toward goals on their individualized course of treatment addressing their unique desires and needs. As a Physical Therapist you will be able to act as a care manager and will work in a team oriented enviroment, while overseeing and insuring a comprehensive rehabilitation program designed to meet the physical, psychosocial, and immediate environmental well being of the client.
This includes promoting client/caregiver ability for self determination and care, optimizing their participation in this process and optimizing outside resource utilization to achieve self management. The Physical Therapist is responsible for coordination and communicating with various disciplines or services involved in client care and coordinating their efforts toward discharge goals established at onset and during course of care. Other information:
Bachelors degree and educational requirements needed to be licensed as a Physical Therapist in New Mexico and to meet requirements of Home Health Medicare Conditions of Participation if necessary. One year experience in supervised clinical practice in acute care or rehab setting preferred. Basic typing competency preferred. Need to have current drivers license as well as auto insurance

Responsibilities:
*Evaluates patients, develops plans and goals, and implements treatment programs for improvement of physical or developmental dysfunction and independence in activities of daily living. Continuously reassesses treatment plans and goals.
*Evaluates, observes and instructs clients on work related functional skills. Educates and instructs regarding home programs and preventive measures as appropriate to patient, family, and others.
*Initiates documentation in the home and completely records all care delivery as required by the profession and the department by the end of each day; this includes admissions.
*Communicates with patient, family, physician, and other healthcare providers regarding patient care.
*Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
*Participates in educational programs and inservices and other meetings as required. Participates on special project teams and other related duties as assigned and requested.
