As a Medicare Part B Mobile Therapy Provider, we offer comprehensive home-based physical therapy services to those who need them most. Beyond insurance-based services, we also provide Cash Based Services to ensure that all patients have access to the top-tier care they require. Each therapy session is scheduled for a full 60 minutes, allowing our therapists to maximize therapeutic benefits and provide undivided attention to each patient.
The Role
As a Home Health Physical Therapist with [Your Company Name], you will be part of a dedicated team focused on providing the best possible care to patients in their own homes. This role allows you to truly set your own schedule, offering the flexibility that is often hard to come by in traditional outpatient settings.
Key Responsibilities
- Provide physical therapy evaluation and treatment in patients' homes
- Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
- Adjust and update treatment plans to meet patient needs and progress
- Document patient care services by charting in patient and department records
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in professional societies

Qualifications
- Current state licensure or certification as a Physical Therapist
- Minimum of one year of physical therapy experience preferred
