Job description

Job Description: The individual hired for this position provides on-site and community-based crisis intervention and follow-up services during weekly shifts as well as participates in an on-call shift rotation, as part of the 24-hour, 7 day a week, Chemung County Crisis Program based at Family Services of Chemung County, Inc. (FSCC), and serving Chemung and Schuyler Counties.

Responsibilities:

  • Provides on-site and community-based in-person and telehealth—telephone, video and face to face—crisis intervention screening, for individuals/families with same day/next day treatment referral and follow-up including complex care management.
  • May participates in an on-call shift rotation in addition to scheduled weekly shifts.
  • Responds to emergent client needs, including risk and other safety assessments and referral for further evaluation.
  • Engages clients from a person-centered, strength-based, trauma-focused, recovery-oriented holistic perspective, addressing social determinants of health.
  • Assists in developing a plan of care with individuals in association with all community providers, family, and collateral supports identified by and for the individuals.
  • Responds to a range of clients’ needs using a variety of intervention methods, including de-escalation and dispute resolution techniques.
  • Provides consultation and facilitation with other staff regarding emergent client situations requiring brief, complex, and per diem crisis intervention.
  • Must be capable of determining when further assistance from the Licensed Mobile Crisis Specialist and/or Supervisor is needed.
  • Completes referrals to community resources and clinical services.
  • Completes timely documentation.
  • Maintains clinical records and effectively uses the established electronic health record in accordance with established clinic policies and procedures and applicable regulatory requirements.
  • Participates, engages, listens, and learns in individual and team meetings.
  • Evaluates individual position and program operation and communicates identified needs to supervisor.
  • Reviews and adheres to all agency policies and procedures.
  • Monitors compliance with New York State Office of Mental Health (NYS OMH)

regulations and guidelines.

  • Complies with NYS OMH and NYS Office of Child & Family Services (OCFS)

Mandated Reporter responsibilities.

  • Maintains and develops current professional skill-base appropriate to position.
  • Completes all program and agency training requirements.
  • Provide community-based training as needed.
  • Represents the agency in the community and maintains relationships with community partners with courtesy and professionalism.
  • Supports a culture that prioritizes professionalism, kindness, teamwork, and staff development.
  • Contributes to the development of new programs and initiatives, and the pursuit of available funding for new or existing programming.
  • Other assignments and tasks may be added at the discretion of the supervisor

Requirements:

Applicants are expected to possess at least a Bachelor’s Degree in a Mental Health-related discipline with at least one to three years of mental health-related experience.

Applicants must have a valid NYS driver’s license and a means of transportation in order to provide Mobile Crisis intervention services to various community sites.

The ideal candidate will demonstrate strong verbal and written communication skills, and problem-solving skills. This individual also will possess an ability to work independently as well as in a teams-based approach with others. This individual will be organized, detailed oriented, and able to respond quickly to community and individual needs.
