DESCRIPTION:
The Mental Health Worker will work with Housed Adult consumers who suffer from severe and persistent metal illness and substance abuse issues.
Mental Health Worker provides comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral to community and social support services through person-centered care planning that incorporates cultural appropriateness. Fulfills case management services which include coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, establishing community supports, supportive counseling, crisis intervention, and providing transportation as needed. Mental Health Worker will be responsible for the assessment of consumers throughout enrollment in program. Mental Health Worker will be responsible for the assessment of consumers throughout enrollment in program. Mental Health Worker will collaborate with family and health care professionals to develop Individualized Rehabilitation Plans that are consumer centered. Mental Health Worker will also works with administrative staff to ensure that all services are documented based on billing and regulatory standards.

REQUIREMENTS:
Bachelor’s Degree in helping profession and with 1 year of relevant, post degree experience. Valid NJ Driver’s License required. Must be computer literate, organizational ability, excellent communication and interpersonal skills are required. Bilingual (Spanish or ASL) preferred, but not required.
