• $55,000.00/yr. - $62,600.00

Sign-on Bonus Available:

• Unlicensed Clinicians: $5,000.00
• Licensed Clinicians: $7,000.00

Position Summary:

The Transitional Age Community Treatment (TACT) program is a community and home-based treatment provided to clients aged 15-29 using a collaborative team approach. The TACT program provides a supportive environment to help clients identify their goals toward recovery and resiliency. TACT teaches skills that will help clients work towards their self-identified life goals (social, educational, housing, benefits, illness management, etc.).

The TACT team is staffed with well-trained and compassionate staff, including a program director, therapists, vocational support, family support, and peer support specialists who receive the training necessary to help clients and families navigate this potentially scary time of life.
 

Duties Performed:

• Provide person-centered assessment, goal planning, and treatment for Mental Health and/or substance use issues. Provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines.
• Develop strength-based written treatment plans/updates and continuing care plans and make appropriate referrals. Complete written discharge summaries and correspondence within established timelines.
• Observe clients in a variety of situations. Assess diagnosis and mental status of clients, evaluate current level of functioning, and develop and implement treatment/service plan to address clients' needs.
• Intervene and de-escalate crises with clients. Provide crisis intervention via telephone or face-to-face in a variety of settings that may include crisis walk-in services, mobile on-site services, residential settings, etc. Assess the risk of harm to self or others and arrange needed resources.
• Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to provide services to clients. Maintain close and cooperative relationships with community agencies and other public agencies in carrying out treatment/service plans.
• Collaborate continuously with assigned clients to devise safety planning, provide symptom management, identify support systems, and reduce and mitigate risk factors. May deliver services to clients in their homes or other settings in the community.
• Assist individuals in improving their well-being and quality of life by training in behavioral skills; educate clients to make informed choices; develop clients' coping skills; help clients to problem solve and negotiate tasks of daily living.
• Learn, utilize, and support fidelity of any evidence-based practices within the assigned program to ensure effective client interventions.
• Perform case management services as needed to assist clients to meet their goals as stated in client service and/or treatment plans. Refer clients to internal and external supportive services such as vocational, medical, social services, and housing authority as well as residential, respite, and inpatient treatment.
• Maintain electronic health records following agency standards; confirm coding of services rendered is ethically appropriate and results in successful billing. This may include entry into a partnering agency's electronic systems.
   

Education/Credentials:

• A Master's Degree in psychology, social work, or another behavioral health discipline from an accredited college or university is required.
• A Valid Driver's License and driving record that meets agency insurability requirements is required.
• Bilingual Skills (English/Spanish) are desired.

• No experience is necessary but is preferred

Salary Placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary placements generally occur between the starting wage and the midpoint of the salary range. 

Employees may be eligible for additional pay based on program, evening, night, weekend, or holiday work. Bilingual pay may be available upon successful passing of language validation testing.
