A Brief Overview
With the System Director of Pharmacy, will develop strategies and processes throughout the healthcare system to avoid, prevent and ameliorate adverse outcomes or injuries that result from the medication-use process. Will provide services to patients from birth to death.
Education Qualifications

  • Bachelor's Degree Graduate of an accredited school of pharmacy. Required or
  • Doctorate Doctor of Pharmacy (PharmD). Required

Experience Qualifications
  • 5 years Experience in health system or ambulatory pharmacy setting. Required or
  • 2 years Experience in health system or ambulatory pharmacy setting and PGY1 or PGY2 pharmacy residency. Required

Licenses and Certifications
  • Pharmacist - KBP Pharmacist - KBP Licensed or eligible for licensure in the state of Kansas. Must obtain active license within 90 days of employment. Failure to obtain active licensure within designated timeframe will result in termination. Required

What you will do
  • Serves as the healthcare system resource on Medication Safety and chair of the Medication Safety Committee whose duties include setting the agenda, reviewing medication error reports, and examining the progress of projects and initiatives assigned to the Medication Safety team.
  • Participates in or leads multidisciplinary teams concerning medication errors, adverse drug events and reactions, near misses, policy review, safe medication use, new product review, and patient safety.
  • Ensures compliance with state and federal Medication Safety regulatory and legal requirements and applicable medication management standards and NPSGs.
  • Collects, reviews, and analyzes the organization's medication-use process, medication errors, adverse drug reactions, and continuous quality improvement data to identify improvements. Utilizes data driven performance-improvement methodologies and tools (e.g., FOCUS-PDCA, Lean methodologies, RCA, FMEA) to develop recommendations for action.
  • Proactively identifies risk points in the medication use-system by utilizing medication event-reporting data, practice standards, best practice guidelines (or recommendations), external error reporting, and Medication Safety tools to reduce the potential for medication errors and patient harm.
  • Leads the review, evaluation, and root cause analysis of significant medication variances and recommends process/system changes. Collaborates with the risk management team and the pertinent healthcare providers to formulate and follow up on plans to address the suggestions for change.
  • Develops and executes a strategic Medication Safety plan in both the short- and long-term.
  • Develops and leads implementation of high-leverage error-reduction strategies to minimize the potential for medication errors associated with medication labeling, packaging, nomenclature, storage, standardization, preparation, distribution, dispensing, and administration.
  • Encourages system-wide medication error reporting. Fosters a culture of safety through "lessons learned" education and communication across the entire organization.
  • Provides researched, accurate, and timely responses to safety concerns presented by safety, quality, pharmacy, nursing, medical staff, and other disciplines and specialties.
  • Provides Medication Safety education and presentations on safe medication use to diverse audiences (pharmacists and technicians, nurses, respiratory therapists, and physicians).
  • Collaborates with IT and pharmacy informatics to provide input in ensuring technology is supporting safe medication practices.
  • Maintains knowledge of trends and developments in the patient safety field through organizations such as ISMP and other continuous professional development opportunities.
