General Summary/Responsibilities:
SOCIAL SERVICES
SHIFT: Full-Time
HOURS PER PAY: 80 hours pay period
The role of the Medical Social Worker involves delivering direct social work intervention, conducting psychosocial assessments, engaging in transition planning, and counseling patients and their families. The Medical Social Worker assesses various aspects of the patient's situation, including social, emotional, financial, cultural, and family considerations. This information is then communicated to the physician, nursing staff, and other care team members to facilitate appropriate transition planning. The Medical Social Worker also assists patients and families in establishing connections with community resources and offers consultation for those seeking input and guidance to address patient concerns. Transition planning and social work services are extended to the hospital's inpatient and outpatient populations.
Qualification
Education:
- Master’s Degree in Social Work required.
Licensure:
- Licensure with the State of Ohio Counselor, Social Work, and Marriage and Family Therapist Board. LSW required, LISW preferred.
Certification:
- None required
Experience:
- Previous experience in a health care setting or other social service agency is required. 3-5 years of experience is preferred.
Skills/Other:
- Must possess strong clinical and interpersonal skills.
- Must have strong communication skills, both verbally and in writing.
- Ability to work in a busy, stressful environment and work independently.
- Current knowledge of medical terminology, Medicare/Medicaid, and third-party payor requirements preferred.
- Basic Computer Skills required.
