Form of workMedical Social Worker works in conjunction with the nurses to provide support services to the patient and family. They should also model appropriate behavior as exemplified in the HCHC operating mission statement which reflects commitment to the Continuous improvement process. Evaluates patients significant social and emotional factors related to health problems and performs periodic re-evaluations.
This position is responsible for Discharge Planning, in home set ups, documentation for MDS and will cross train in Admissions.
Qualifications
EDUCATION & TRAINING: Bachelor of Science from an accredited four-year program of Social Work.
EXPERIENCE: Minimum one year hospital experience preferred.
1. Knowledge of social work interviewing, counseling, and assessment.
2. Knowledge of social/medical/psychological issues relating to all age groups.
3. Knowledge of community resources.
4. Ability to learn computer systems and documentation software.
5. Communication skills--verbal, reading, writing.
6. Organizational skills--attention to detail, accuracy and ability to follow instructions.
7. Professional appearance and personality.
8. Ability to elicit cooperation and maintain good working relationships among personnel.
9. Knowledge of commercial/government payor sources and utilization/ precertification requirements.
LICENSES & CERTIFICATION: Has acquired or working to acquire current licensure in the state of Tennessee. (NOTE: Education and experience shall meet the standards for qualified Social Worker as defined by JCAHO.)
