Medical Records Clerk/Admissions Coordinator
The Medical Records personnel compile, process, and maintain Medical Records of the nursing home in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the health care system. They process, maintain, compile, and report patient information for health requirements and standards. They are responsible for safeguarding patient records and performing other clerical tasks such as answering phones, faxing, and patient check in and out.

They process, maintain, compile, and report patient information for health requirements and standards. They are responsible for safeguarding patient records and performing other clerical tasks such as answering phones, faxing, and patient check in and out.
The admissions coordinator holds shared responsibility for census development, timely and efficient management of all incoming referrals, and ensuring smooth transitions during the admissions process. In conjunction with the Admissions Director, the Admissions Coordinator manages the facility inquiry and admission process and provides customers with facility-related information via facility tours, personal visits/assessments, conversations and follow-up. They assist with and participate in marketing initiatives as requested. They serve as a friendly and professional representative of the organization in all interactions.
Responsibilities:
- Initiates the medical chart and is responsible for routing the chart to the appropriate personnel
- Maintains patient files and retrieves files as indicated
- Files patient charts; files all patient data upon receipt of information; initiates records for new patients; prepares file labels; maintains filing statistics; audits filing sequence
- Responds to requests for Medical Records; processes letters and reports; answers and directs telephone calls
- May photocopy records and documents for billing and/or legal services; sends and receives information via fax/email
- Keeps supervisor informed of problems or issues; monitors Supplies as needed--'
- Demonstrates organizational skills
- Demonstrates telephone etiquette skills
- Releases information to persons and agencies according to regulations
- Protects the security of Medical Records to ensure that confidentiality is maintained
- Maintains and tracks Audiology, Dental, Optometry, and Podiatry visits and residents who are due to be seen
- Thins out residents' charts as necessary
- Covers receptionist's lunch break
- Conducts tours and follow-up as needed
- Daily interaction with Admissions Director; serves as liaison between Admissions Director and facility; Communicates bed availability to Admissions Director
- Alerts appropriate department heads and building staff of projected changes; i.e., admissions, bed changes, and discharges daily through established communication systems; communicates with nursing and housekeeping about room and bed selection
- Ensures that appropriate admission paperwork, signatures, and photos are obtained from residents or responsible parties
- Communicates special needs of new admissions to staff to ensure a smooth transition
- Maintains working knowledge of Federal and State regulations and reimbursement guidelines (Medicare and Medicaid)
- Assists with managed care referral and authorization process
- Keeps all patient information confidential
- Signs in all patients at point of admission
- Performs other duties as assigned
