Position Summary: The Medical Records Clerk ensures the safety, security, and confidentiality of resident information while maintaining resident MEDICAL RECORDS and health information systems in accordance with current federal and state guidelines.

Duties/Responsibilities:

  1. Receive and follow work schedule/instructions from your supervisor and as outlined in our established policies and procedures.
  2. Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility, to assure that MEDICAL RECORDS can be properly maintained.
  3. Files information such as progress notes, laboratory reports, x-ray results, correspondence, etc., into resident charts.
  4. Collect, assemble, check and file resident charts as required.
  5. Ensure incomplete records/charts are returned to appropriate departments or personnel for correction.
  6. Assist in developing procedures to ensure resident records are properly completed, assembled, coded, signed, indexed, etc., before filing.
  7. Establish a procedure to ensure resident charts/records do not leave the MEDICAL RECORDS room except as authorized in our policies and procedures.
  8. Maintain a record of authorized information released from charts/records, i.e., type information, name of recipient, date, department, etc.
  9. Collect charts, assemble them in proper order, and inspect them for completion.
  10. Pick up and deliver resident MEDICAL RECORDS from nurses’ stations and other designated areas as necessary.
  11. Answer telephone inquiries concerning MEDICAL RECORDS functions. Prepare written correspondence as necessary.
  12. Retrieve MEDICAL RECORDS when requested by authorized personnel (i.e., physicians, nurses, government agencies and personnel, etc.)
  13. Assure that MEDICAL RECORDS taken from the department are signed out and signed in upon return to the department (e.g. MEDICAL RECORDS or storage room).
  14. File active and inactive records in accordance with established policies.
  15. Index MEDICAL RECORDS as directed.
  16. Screens employees and visitors in and out of the facility.
  17. Serves as back up for maintaining residents’ trust funds.
  18. Serves as back up to the Nursing Secretary with the daily census.
  19. Serves as a backup in the absence of the Receptionist.
  20. Adhere to all HIPAA guidelines.
  21. Performs other related duties as assigned.

Required Skills/Abilities:

  • Highly detail oriented and organized in work.
  • Ability to act with integrity, professionalism, and confidentiality.
  • Ability to multitask and work independently with minimal daily direction from manager to accomplish objectives.
  • Ability to meet assigned deadlines.
  • Excellent interpersonal, communication and problem-solving skills.
  • Must be able to read, write, speak, and understand the English language.
  • Knowledgeable of medical terminology.
  • Proficient with Microsoft Office Suite (specifically Microsoft Outlook, Excel, and Word) or related software.
  • Able and willing to deliver friendly, courteous, and prompt customer service.
  • Able and willing to work cooperatively with other team members.
  • Ability to handle medical and financial information with the highest degree of confidentiality and sensitivity.

Qualifications:

  • High school degree or GED required.
  • Associate degree in business administration or social services preferred.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.

Monday - Friday (6:00 AM - 2:30 PM)
