Responsibilities:
- Maintain current knowledge of record management systems through cross trainings of all aspects of records management and client data entry. Be able to perform other duties in Medical Records as needed.
- Maintain bottom line responsibility for delegated tasks such as maintenance of electronic Medical Records, copying of forms, destroying confidential material, securing and accessing client records.
- Communicate with staff and clients and external agencies/providers and facilitate smooth operations.
- Assist with the mailing of closure pending transition plans, discharge plans and final closure notification to designated contacts. Track responses from closure follow up surveys.
- Assist with updating/entering diagnosis from data we receive from external documentation for PRP/RRP and CEP. Diagnosis update/referral form is required every six months.
- Assist with updating and verifying emergency contacts.
- Scanning documents and attaching to the client’s electronic medical record.
- Opening and closing client records
- Requesting documentation and processing request from external providers.
- Searching for information in client record for staff that lack access rights.
- Maintaining the electronic Medical Records.
- Perform initial enrollment audits of designated programs.
- Take and maintain SRT meeting minutes
- Enter all open/close tickets onto the electronic Medical Records.
- Assist Compliance and Quality Improvement as needed.
Requires:
- Possession of a high school diploma or GED.
- Two years positively referenced relevant work experience.
- Valid driver’s license or alternative transportation plan.
- Willingness to learn and apply mental health service delivery concepts as they relate to interactions with members.
