As part of our PACE program, you will serve as both the health insurer and primary care provider for our patients, allowing you to truly engage with and care for them without the constraints of fee-for-service incentives. Reporting to the Chief Clinical Officer, the Medical Director will play a central role within the IDT, contributing to participant care.
Essential Job Duties:
- Conduct comprehensive assessments for participants and collaborate with the IDT to create individualized care plans.
- Actively participate in the IDT, facilitating communication regarding participant changes, care planning arrangements, and 24-hour care coordination.
- Collaborate with network providers and fellow IDT members to ensure efficient care coordination, especially concerning hospital discharges and short-term/long-term placements.
- Provide guidance and supervision to Nurse Practitioners.
- Oversee participants' utilization of medical specialists and inpatient care.
- Share in rotational night and weekend on-call responsibilities.
- Supervise all aspects of participant care in nursing homes, conducting regular visits in accordance with PACE standards and participant needs.
- Review outcome and utilization data, including adverse events, hospitalizations, and specialist utilization, in partnership with the IDT to identify opportunities for enhanced care and operational efficiencies.
Job Requirements:
- Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Master’s or doctorate degree.
- Board Certification in Internal Medicine or Family Practice.
- Preferred Board Certification in Geriatrics.
- Active California Physician License without restrictions.
- State CDS Certification and DEA Registration.
- Valid state driver's license with a clean DMV record and recent vehicle insurance.
- Excellent clinical, organizational, and communication skills, especially when working with frail, elderly participants, their families, and the IDT.
- Up-to-date knowledge of chronic care and geriatric issues, along with best practices.

The base salary/wage range for this role is $300,000 - $330,000, accompanied by a bonus and equity opportunities. The actual compensation will be determined based on experience and qualifications.
