Responsibilities:
- Provide medical oversight and direction for the hospice program, including implementing clinical policies, procedures, and protocols.
- Participate in the admission and certification process for hospice patients, reviewing medical records and collaborating with the interdisciplinary team to determine eligibility and develop individualized care plans.
- Conduct regular assessments of patients' medical needs and goals of care, collaborating with attending physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive and coordinated care.
- Lead interdisciplinary team meetings to review patient cases, discuss treatment options, and address any changes in condition or care plans.
- Provide consultation and support to attending physicians, nurse practitioners, and other healthcare providers involved in the care of hospice patients, including pain and symptom management, advanced care planning, and end-of-life decision-making.
- Educate and train hospice staff members on medical issues, palliative care principles, pain management techniques, and end-of-life care best practices.
- Serve as a resource for staff members and families regarding medical questions, treatment options, and ethical dilemmas related to hospice care.
- Collaborate with community healthcare providers, hospitals, long-term care facilities, and other organizations to enhance access to hospice services and promote continuity of care.
- Ensure compliance with regulatory standards, accreditation requirements, and quality improvement initiatives related to hospice medical care.
- Participate in performance improvement activities, including clinical audits, peer review, and outcome measurement, to assess and enhance the quality of care provided by the hospice program.
Qualifications:
- Medical degree (MD or DO) from an accredited medical school.
- Board certification in Hospice and Palliative Medicine preferred.
- Current medical licensure and DEA registration in the state of practice.
- Minimum of three years of clinical experience in hospice, palliative care, or related field.
- Strong leadership and communication skills, with the ability to collaborate effectively with interdisciplinary teams, patients, families, and community partners.
- Knowledge of end-of-life care principles, pain management, symptom control, and advance care planning.
- Commitment to compassionate, patient-centered care and respect for patients' autonomy, dignity, and cultural preferences.
- Ability to adapt to changing priorities, work independently, and manage multiple responsibilities in a dynamic healthcare environment.
