Medical Clinician Team Lead- Family Medicine

A Medical Clinician Team Lead represents and leads the clinical activities for the assigned care team units in collaboration with the Practice Manager and/or Supervisor. The Medical Clinician Team Lead is responsible for the overall direction and daily performance of the clinicians assigned to their Care Team Unit. Individuals chosen for this position are expected to have the highest standards for clinical capability, professional education, quality improvement, staff accountability, clinical performance, customer service, and overall patient care. Additionally, Medical Clinician Team Leads should have the wisdom and judgment to guide One Community Health's programs and professional staff in pursuit of superior patient care. The Medical Clinician Team Lead, together with the Practice Manager and/or Supervisor, will be responsible for the Care Team's daily overall performance and functions inclusive of scheduling, direct patient care, preventive and follow-up care, patient satisfaction and process improvement activities.

This role is on-site in Sacramento, CA (95811).

ESSENTIAL FUNCTIONS

Administrative

  • Assist with recruitment and retention of high-quality licensed clinicians for the Care Team in collaboration with the Division Medical Director and/or Assigned Medical Director.
  • Lead the retention activities for the clinicians for the Care Team and maintain a high level of clinical provider engagement and satisfaction.
  • Ensure the orientation and mentoring of new clinicians to the Care Team unit.
  • In collaboration with the Division Medical Director, assist with the periodic evaluation of the performance of the clinical members of the Care Team.
  • Co-lead daily Huddles with Practice Manager and/or Supervisor to review and address clinic priority activities of the day including staffing and scheduling.
  • Conduct regular care team meetings in collaboration with the Practice Manager and/or Supervisor.
  • Disseminate information to clinicians and staff members of the care team in a consistent and timely way.
  • Participate in service line and other clinical program development.
  • Together with Medical Leadership, co-lead with the Practice Manager and/or Supervisor continuous quality improvement efforts for the divisions.
  • Together with the Practice Manager and/or Supervisor analyze and address variances between the care team's actual versus budgeted profit and loss statement.
  • In collaboration with the Practice Manager and/or Supervisor, manage absences (CME, paid time off, sick leave, etc.) for the care team clinicians and help to ensure sufficient access (50% or more of the clinicians should be available on any given day) for patient care at all times and assist Division Medical Director with coordination of provider coverage for prolonged absences (sick leave, vacation, CME, etc.).

Clinical

  • Provide comprehensive primary care to adult patients, including preventive medicine and management of chronic diseases.
  • Skillfully diagnose and treat acute illnesses, as well as chronic diseases and conditions such as hypertension, heart disease, and diabetes, employing your expertise to optimize patient outcomes.
  • Clearly communicate with patients about their treatment plans, thoughtfully explaining procedures, test results, and prescribed therapies, fostering understanding and trust.
  • Attentively track patients' conditions and progress, adapting treatments as necessary and ensuring optimal care at every stage of their journey.
  • Exercise sound judgment in prioritizing medical care, collaborating with specialists as needed to address high-risk or complex cases, ensuring comprehensive support for each patient.
  • Uphold a steadfast commitment to excellence, ensuring top-quality medical care is delivered to all patients and fostering a culture of continuous improvement.
  • Demonstrate a thorough understanding of medical clinic policies and relevant state and local regulations, ensuring compliance, appropriate patient follow-ups, and specialty referrals.
  • Understands and monitors systems for compliance as necessary.
  • Provides necessary and timely communication to Medical Leadership when issues of compliance arise and complies with a corrective strategy.

Additional Duties

  • Leads public health and hazards management activities for the Care Team in collaboration with the Practice Manager and/or Supervisor.
  • In collaboration with the Practice Manager and/or Supervisor participate in the development and maintenance of the annual care team strategic and quality improvement plan.
  • And other duties as assigned.

SUPERVISORY RESPONSIBILITIES

The Medical Care Team Lead coordinates the activities of the clinicians assigned to that Care Team Unit in collaboration with the Division Chair. Supervisory authority rests with the Division Chair.

MINIMUM REQUIREMENTS

  • Must be fully licensed to practice in the state of California, MD/DO
  • Board certified or board eligible MD/DO
  • Possess and maintain a DEA certificate
  • Active BLS Certification.
  • Licenses required for this position must be kept current, valid, and in good standing with the presiding entity.
  • Driver's License- possible travel between locations

PREFERRED BACKGROUND

  • Experience with clinical operations and medical leadership.
  • Understanding of PCMH principles and practices.
  • Background in quality improvement.
  • Population health management knowledge.
