Knowledge and Skills:
Professional and timely verbal (in-person/telephonic) and nonverbal communication.
Time management including prioritizing multiple tasks and timely completion.
Computer literacy and thorough documentation.
Working knowledge of behavioral health symptoms, presentations, diagnoses and treatments.
Positively interact with patients experiencing chronic or acute mental illness including de-escalation, crisis management, safety planning and rapport building.
Accurately collect, document, and track patient data.
Researching and contacting resources and community partners to build and maintain a functional referral source base.
Duties:
Coordinate needs of patients and appropriate levels of care with stakeholders.
In-person, telehealth and/or telephonic patient support services.
Triage patients to assess for safety and resources needed.
Assist patients with FMLA, disability and insurance benefits, work restrictions paperwork, phone calls, referrals, assistance programs, etc.
Assist others with patients with acute needs and coordinate with the treatment team.
Act as a liaison between PCP and behavioral health provider(s) to promote integrated care.
Outreach patients to support continuity of care as needed.
Complete assessment screenings for Social Determinants of Health (SDOH) and Health (HRA) when applicable.
All medical support including but not limited to vital signs, lab work and pharmacogenetic tests, flowsheets, assessments, records requests, prior auths, injections, drug screens, doc-to-doc setup, med log, more.
Opening and closing duties as assigned
Department Administrative duties (voicemails, phone calls, etc)
Provide samples to patients as ordered by PNP and document in patient’s chart accordingly
Other duties as assigned and skills-building training as needed.
Qualifications and Education Requirements
Bachelor’s Degree plus two (2) or more years of experience in behavioral health including experience within the past 24 months providing medical and or behavioral services, or an equivalent of experience, skills and education.
Medical Assistant Certificate preferred or Phlebotomy, Nursing, EMT or CNA. Certifications must be from a fully-accredited program the entire time of attendance or applicable training otherwise. Must supply copies of degrees and transcripts as requested.
