Medical Assistant/Care Coordinator

Overview:

We are a small company that manages home based primary care practices nationwide.
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the smooth coordination and delivery of healthcare services to patients. This position requires strong organizational skills, medical knowledge, and excellent communication abilities.

Duties:
- Facilitate communication between patients, healthcare providers, and other staff members
- Collect and record patient information, including medical history, medications, etc
- Confirm appointments and prep patient charts prior to visits
- Send orders, referrals, labs and be consistent with follow up
- Ensure accurate documentation of patient records and maintain confidentiality
- Chronic care management
- Collaborate with healthcare team members to develop and implement care plans
- Facilitate communication between patients, healthcare providers, and other staff members

Qualifications:
- High school diploma or equivalent required; associate's or bachelor's degree in healthcare-related field preferred
- Medical Assistant/Care Coordinator experience is highly desired
- Knowledge of medical terminology and anatomy
- Proficient in using electronic health record systems such as Kareo (Tebra), AdvancedMD and/or Carelink experience is a plus
- Excellent interpersonal skills with the ability to communicate effectively with patients and healthcare professionals
- Strong attention to detail and organizational skills
- Ability to multitask and prioritize tasks in a fast-paced environment

- Strong computer skills, including knowledge of Microsoft Office Suite
