Responsibilities
-Establish and maintain clinical relationships with patients
-Conduct regular assessments of chronic conditions
-Develop a Patient Centered Care Plan working with provider.
-Take daily vitals, which will be remotely uploaded to provider
-Provide education, training, and motivation to help patients improve their chronic conditions
-Act as patient advocate to assure optimal care
-Diabetes Education
-Daily Review of patients vitals
-Administer Annual Wellness Visits
-Administer Neurocognitive Testing
- Coordinate patient care activities and ensure smooth operations within the healthcare facility
- Provide medical administrative support by maintaining patient records, and updating information
- Collaborate with healthcare providers to ensure accurate and timely communication of patient information
- Educate patients on their healthcare options and assist in making informed decisions
- Monitor patient progress and follow up on treatment plans to ensure optimal outcomes
- Maintain confidentiality and adhere to all HIPAA regulations
Qualifications
- Certified Medical Assistant
-Previous experience in a healthcare setting preferred
- Knowledge of medical terminology, anatomy, and physiology
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize tasks effectively
- Proficiency in using electronic medical records systems
- Familiarity with medications and basic medical procedures
- Ability to work well in a team environment
