Job Description
The Senior Services Care Coordinator provides support for the senior population relating to healthcare plans (Medicare Advantage, Methodist ACO and Traditional Medicare) for seniors. Provides support to ensure each patient has the opportunity to participate in program requirements. Works closely with the Quality Manager to conduct tasks within the department to include screening tests and completing documentation measures required by the different health plans. Will compile numerous reports for Senior Population data mining. Excel experience is a plus. This position requires heavy patient contact in person and by phone. Clinical:
- Handle incoming inquiries from team members, including providers, CMA's and Triage members
- Conduct Focus of Care Testing on patients, order lab requisitions, scan and record results in the patient charts.
- Inventory medical supplies weekly and submit order to Team Lead.
- Compile Excel reports using insurance websites and mail correspondence.
- Call senior patients to schedule for testing, appointments, and education sessions.
- Desktop support- all items are processed each day.
- Provide coverage for other Senior Care Coordinators when necessary.
- Coordinate training schedules for Providers and team members from outside vendors.
- Send out communication emails to providers, team members and outside vendors.
- Copy educational material for patients, providers and team members.
- Conduct patient attestations.
- Attend meetings with team and outside vendors.
- Travel to the different VHP clinic sites occasionally.
- Other duties as needed.

- Maintain confidentiality of patient and personnel information
- Follow all clinical safety protocols and infection control procedures
Qualifications:
- 3 Years Minimum of experience in direct patient care/ physician office setting
- Certified Medical Assistant- active certification
- Excellent customer service and communication skills.
- Highly motivated and organized.
- Experience with basic desktop software, medical practice management, and electronic medical record systems, Word and Excel
- Knowledge of and adherence to HIPAA rules and regulations.
- Knowledge of and adherence to Bloodborne Pathogen & OSHA Safety procedures
