Job Description
- Collects and records patient data including height, weight, BMI, temperature, pulse, respiration rate and blood pressure, pulse oximeter and peak flow measurement according to established policies and procedures.
- Collects urine, sputum and stool specimens by noninvasive techniques.
- Administers immunizations and medications per provider's order after a licensed health care professional verifies the correct medication and dosage. Verification of competency is required.
- Performs venipuncture to obtain blood specimens.
- Performs simple lab tests, including urine pregnancy tests and finger sticks after appropriate training and verification of competency.
- Schedules routine and follow-up appointments. Contacts no-show patients, as directed.
- Operates a computerized patient scheduling system according to organizational policies and procedures. Collaborates with providers to assure appropriate scheduling.
- Participates in the follow-up and monitoring of patient care services, including (but not limited) checking medical records for completeness of data, documentation of services, completion and mailing of disability forms, verification of hospital of delivery and mailing of pre-natal records, relaying telephone messages to staff personnel, and directing patients throughout the clinic as needed.
- Skill in interviewing and communicating to elicit information and cooperation from patients
- Conducts street outreach activities (that may include extended working hours) with outreach staff of other agencies and indigenous leaders to identify individuals with medical needs and to provide information regarding Care Link and other community resources available
- Promotes appropriate health screening and testing such as prenatal and perinatal health care and/or HIV testing; assists clients, their caregivers and families in developing healthier lifestyles; provides information concerning ways to reduce the risk of transmission of communicable diseases.
- Ability to prepare routine records and reports using terminology common to outreach programs
- Skill in establishing and maintaining effective working relationships with patients, staff, and the public
- Knowledge of community resources and referral agencies
- Ability to communicate effectively in English and Spanish written and verbal, commensurate with high school levels is highly desirable
- Partners with Staff & Community Engagement Coordinator to assist in community outreach program aimed at increasing client support services and community engagement
- Communicates with supervisor regularly concerning any program challenges or new projects
- Maintain a caseload of 12-15 clients
- Marketing experience preferred.
- Must be Bilingual-Spanish
