Job Summary
We are seeking a Medical Assistant to join our team as a Care Coordinator at our long-term care facilities ! As our Care Coordinator, you will be responsible for medical oversight of patients enrolled in our Remote Patient Monitoring Program. Responsibilities will include daily check-ins including vitals, and assuring patients are adhering to medications and diet. You will be responsible for periodic assessments and the establishment of a patient-centered Care Plan. We aim to provide motivation, coaching and training to help our patients improve their chronic conditions. As Care Coordinator, you will also be providing regular education on hypertension, diabetes, and weight loss. You will also be a Patient Navigator, coordinating communication between all the patient's doctors, and be a liaison to the patients family. Full training will be provided.
Responsibilities

• Establish and maintain clinical relationships with patients
• Take daily vitals
• Conduct regular assessments of chronic conditions
• Create and maintain a Patient-Centered Care Plan
• Provide education, training, and motivation to help patients improve their chronic conditions
• Liaison between patients, physicians, and families.
• Maintain our remote electronic health record

Qualifications
