- Participate in training related to the Medicaid Unwind project (on-line or in person).
- Travel throughout Region 2 is an integral part of this role. You may also be asked to travel on-site to the LPCA Baton Rouge office from time to time for trainings or to assist the Medicaid Unwind Project Director, as necessary.
- Sign required agreements related to Medicaid Unwind outreach efforts.
- Contact Medicaid beneficiaries that are currently receiving benefits and may be impacted by the Consolidated Appropriations Act, 2023 which made several changes to Section 6008 of the FFCRA.
- Obtain up-to-date contact information for Medicaid beneficiaries through various means that include phone calls, text messaging, emails, social media and/or other modalities.
- Utilize phone, email, social media, and other activities to reach Medicaid recipients affected by legislative changes.
- Set up outreach tables at participating Region 2 FQHCs on a weekly basis.
- Participate in community events to provide Medicaid outreach services as directed by Project Director. This will include disseminating and tracking gift cards for eligible Medicaid patients.
- Engage with Medicaid patients providing Medicaid enrollment education/demographics update.
- Provide for initial processing of Louisiana Medicaid applications and renewals at community events and through neighborhood canvassing.
- Track and report activities using forms provided by LDH in a timely manner.
- Work with Project Director to identify strategies and processes for meeting project deliverables.
- Identify challenges and barriers that are hindering project success.
- Identify resources and best-practices for quality improvement.
- Research available outreach event opportunities and provide findings to Project Director.
- Participate in weekly check-in and planning meetings with Project Director to discuss the prior week’s activities and updates on efforts made during outreach events.
- Travel to outreach events and other community-based gatherings to educate communities of federal Medicaid changes affecting target population (Medicaid recipients.)
- Compile and submit monthly reports to Project Director in a timely manner.
- Collaborate with other local, regional, and state based organizations that share project mission.
- Collaborate with community-based organizations and other institutions that can assist in project deliverables.
- Track and submit daily activities using a monthly performance tracker that includes project metrics as determined by Louisiana Medicaid.
- Attending monthly meetings for updates, best practices and lessons learned.
- High school diploma or equivalent. A baccalaureate degree, preferred.
- Minimum of 3 years’ experience in community or workforce outreach and development, preferred.
- Community Health Worker certified.
- General computer literacy, experience with Microsoft Office and Windows.
