MDS Coordinator job responsibilities include;
Understand and follow through with the MDS process, including MDS assessment, Triggers and Raps, and Care Planning.
Verify completion and accuracy of MDS prior to submission.
Maintain current PPS/Medicare and Medicaid/Private assessment schedules with proper reference dates for regular distribution to therapies, dietary, social service, activities and nursing.
Demonstrate current knowledge of nursing practices and procedures as they pertain to Long Term Care including state, federal, and OBRA regulations.
Requirements
Registered Nurse (RN) with a valid New Hampshire license
Will train a RN with long term care experience
Minimum 2 years' experience in long term care
Must have strong clinical and communication skills
Must be highly organized with the ability to multi-task throughout the day
