MDS COORDINATOR

The RN MDS is responsible for managing the MDS/RAI care plans. Observes and records symptoms and conditions of the residents. Reviews cases and determines reimbursement care mix levels. Assists the Rehabilitation Services, Wellness, Culinary, and Social Services departments with the MDS process, monitors timeliness of MDS completion, and reports any outstanding departments or issues.
Responsibilities:
- Provide support to interdisciplinary team including education, process implementation, and ongoing support/management of systems
- Provide ongoing education and guidance to improve coding accuracy, workflow organization, and compliance
- Collaborate with facility staff to identify barriers to success impacting quality measures, Five Star rating, reimbursement capture
- Guide the IDT in implementing best practices relating to the RAI process
- Analyze and report on key performance indicators to measure success and proactively identify areas of need
- Maintain and share expertise/knowledge related to reimbursement and quality outcomes
- Work in conjunction with IDT to resolve issues, endorse changes and conduct follow-ups to establish that recommendations are effectively implemented and monitored for appropriateness
- Promote compliance by performing periodic audits of assessments, supporting documentation, and other relevant data
- Work closely with DON to achieve facility goals and ensure data is captured
Qualifications:
- Current RI licensure and in good standing
- Preferable RN
- Will consider LPN with below qualifications
- Experience with multiple payment models (PDPM, LTC Case-Mix reimbursement)
- Microsoft Office proficiency required, with the aptitude to learn new programs as needed, being computer savvy is necessary
- Highly motivated and detail oriented professional with excellent analytical and organizational skill set
- Capable of working independently and as part of a team
- Adept skillset for multi-tasking, sense of urgency, ability to prioritize
- Ability to effectively communicate with a wide range of individuals, professional demeanor is imperative
Qualifications Required
- Current state licensure as a Registered Nurse (RN), in good standing.
- Experience in geriatric nursing and LTC.
- CPR certified.
- Knowledge of assigned state and federal regulations governing long-term care facilities.
- Proficient in Microsoft Office and MatrixCare.
- Knowledge of MDS, Medicare, assessment and care-planning process.
- Ability to effectively communicate with residents, families, and interdisciplinary team.
- Ability to maintain confidentially (HIPAA).
- Ability to read, write and communicate in English.
Qualifications Preferred
- MDS/RAI experience.
- AANAC Certification.
