Position Overview:
Harmony Health Care Management is seeking a dynamic and results-driven Community Relations Specialist to join our team. Reporting to the Marketing Director, the Community Relations Specialist will play a crucial role in building and maintaining positive relationships with key stakeholders, including hospitals, doctors' offices, healthcare professionals, local organizations, churches, and other community partners. The primary focus of this role is to drive patient referrals by engaging with healthcare providers and community organizations.
Key Responsibilities:
- Collaborate with the Marketing Director to develop and execute comprehensive Community Relations strategies and initiatives aimed at increasing patient referrals for Harmony Health Care Management's hospice company and senior living facilities.
- Cultivate and maintain strong relationships with hospitals, physicians, nurses, case managers, social workers, and other healthcare professionals to promote our services and educate them about the benefits of our hospice and senior living programs.
- Identify and attend networking events, conferences, and meetings to connect with healthcare professionals and community leaders and represent Harmony Health Care Management as a trusted partner in care.
- Coordinate and facilitate educational presentations, lunch-and-learns, and in-service trainings for healthcare providers and community organizations to enhance their understanding of our services and referral processes.
- Serve as a liaison between Harmony Health Care Management and external partners to address questions, concerns, and opportunities for collaboration, ensuring seamless communication and coordination of care.
- Collaborate with internal teams, including marketing, admissions, clinical, and operations, to develop marketing materials, presentations, and collateral tailored to the needs of different audiences and referral sources.
- Track and analyze referral trends, market data, and competitive insights to inform strategic decision-making and identify opportunities for growth and expansion.
- Maintain accurate records of outreach activities, contacts, and outcomes in the CRM system and provide regular reports to the Marketing Director on performance metrics and goals.

Pay: $48,000.00 - $100,000.00 per year
