Title: Patient Care Coordinator, Logistics
Department: Patient Care Coordination
Qualifications:
- Doctor’s office assistance experience preferred (Can include any variation of the following experiences: front desk reception, appointment coordinators, benefits & eligibility verification, case management, hospitality management, etc. )
- Invested in positive customer experiences (Customers include: Patient, Payer, Provider, and team)
- Excellent verbal and written communication skills
- Excellent rapport with patients and providers
- Good Morale (attitude)
- Strong compliance with patient, provider, peer, employer, and trade confidentiality
- Ability to effectively multi-task
- Ability to work After Hours and on-call as needed
- Computer proficiency and literacy (Microsoft office suite, EMRs and CRMs)
Responsibilities:
Patient Care Coordinators (PCCs) are responsible for coordinating the services of Patient Care through our network of providers. Duties include, but are not limited to:
- Registration and communication with patients, providers, and payors.
- Plan of Care (POC) and Request For Quote (RFQ) documentation and maintenance under the direction of an assigned medical director and/or Case Manager.
- Appointment scheduling, coordination, and follow-up
- Regular coordination and case maintenance with relevant parties, including but not limited to patients, families, payers, referring and receiving providers, etc.
- Following outlined workflows and procedures during the continuum of Patient Care as outline in the PCC
Departmental manuals and trainings.
- Real time and accurate documentation in JIPA’s proprietary EMR/ CRM system.
- Periodically verify the status of all clinical activities in compliance with treatment plan and JIPA’s criteria as specified in the PCC manual.
- Nurture the patient relationship to encourage patient retention
- Work as team player to ensure each customer receives the best service possible
- Utilize all tools for correspondence with patients, providers relevant parties, including but not limited to phone, fax, email, PFS, etc.
- Submit completed and timely paperwork necessary to perform the responsibilities of the Patient Care
Coordinator;
- Other duties may be assigned as necessary to ensure effective workflow and efficiency
- Ensure that all documents submitted on behalf of the patient are to sent, received, and confirmed in a timely manner to the appropriate health care providers, and must abide by all compliance standards.
- Effectively utilize PFS for all tasks and documentation, by inputting accurate data and update as necessary.
- Participate in regular meetings & trainings
- Abide by office policies & procedures
- Abide by the JIPA professional code of ethics
- Reporting all incidents & liability concerns, including but not limited to: Clinical risks, financial risks,
Confidentiality risks, and patient, provider, or payer satisfaction.
- Regularly maintain patient appointment schedules and reports. This includes but is not limited to confirming appointment with patients and providers, and following up upon completion of the appointment.
- Regular clinical and financial reconciliation of patients’ services and cases.
- Participation in Global and targeted patient censuses as deemed appropriate by the management team.
- Daily prioritization of tasks for emergent, urgent and other coordination services.
- Collection and/or confirmation of documentation including, but not limited to demographic data, medical records, authorizations, and consents.
- Coordinate the order and delivery of pharmaceuticals, DMG and services for patients.
- Provide support in tasks as needed.

Pay: $53,000.00 per year
