Job Description:

As a Home Health and hospice Licensed Vocational Nurse (LVN) at Varco , you will play a pivotal role in delivering exceptional healthcare services to patients within the comfort of their residences. Working collaboratively within a healthcare team, you will conduct in-home health assessments, administer treatments, and ensure the implementation of personalized care plans.

Responsibilities:

1. In-Home Health Assessments:

   • Conduct thorough in-home health assessments to evaluate patients' vital signs, medication adherence, and overall health status.

2. Care Plan Development:

   • Work closely with the healthcare team to develop individualized care plans that address patients' specific needs and goals.

3. Medical Administration and Procedures:

   • Administer medications, treatments, wound care, and medical procedures following prescribed protocols and care plans.

4. Monitoring and Documentation:

   • Monitor and document changes in patient condition, promptly communicating updates to the healthcare team.

5. Patient Education:

   • Educate patients and their families on disease management, medication adherence, and lifestyle adjustments to promote overall wellness.

6. Collaboration:

   • Collaborate with various healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure a holistic approach to patient care.

Requirements:

• Current LVN license in the state of Texas.
• Proven experience as an LVN, preferably in a hospice or home health setting.
• Proficiency in clinical assessment and patient care.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a healthcare team.
• Valid driver's license and reliable transportation.
