Lvn Home Health For Shift Care Nurse- Urgent- New Grads Welcome

Care Circle HH has an URGENT NEED for SHIFT CARE LVN's for patients in Baldwin Park, CA.

Schedule of shift: Mon, Tues, Wed and Thurs: 6-8 hours per shift

This position is open to START IMMEDIATELY and YOU SHOULD BE ABLE TO ACCEPT and TAKE PATIENTS ASAP.

Care Circle Home Health is a Joint Commission-accredited Home Health agency servicing various counties in SoCal.

We offer flexible schedules, training, complete PPE’s and competitive pay. We also use user-friendly EMR/web-based documentation and we concentrate patient assignments close to our nurses’ homes to minimize time in traffic.

LVN's : at least 1 year nursing experience preferred, but newly grads are also accepted

You will need to submit:

• Current California-issued Professional License
• Certificate for Basic Life Support or Active CPR Card
• Driver's License and continue
• Auto Insurance (with the applicant's name on the policy)
• Physical examination within 6 months or "Fit to Work" doctor's note
• PPD or TB/Chest X-Ray test result
• Flu Shot record
• Proof of Covid Vaccine

IF YOU HAVE ALL THE CREDENTIALS AND QUALIFICATIONS, YOU MAY BE HIRED IN A WEEK!

You will be working with our highly qualified Clinical Management Team (RN Case Managers, Coordinators, dedicated QA Team) to provide quality direct patient care, such as:

• ensure quality and safe delivery of Home Health nursing services
• providing accurate and timely documentation consistent with the plan of care
• providing patient and family/caregiver education and important information relating to diagnosis and plan of care
• actively participating in coordination of Home Health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the clinician and/or clinical manager.
