Department: LTC/SNF | LPN
**Required Certifications: BLS | LPNs: LPN license; IV cert preferred.**
- Type of Facility: SNF/ALF
- # of Beds/Residents: 62 beds. Average census of 30.
- EMR: PCC
- Ratios: 1 charge nurse/shift.
- Required Certifications: BLS | LPNs: LPN license; IV cert preferred.
- Common diagnosis/Types of patients: Accept LTC, skilled, rehab, hospice, & dementia residents. Wound vac,
drains, IV treatments, established trachs (with or without O2 needs), dialysis patients.
- Job Duties: Floor RN and LPN roles are very similar and include: Med and treatment administration, blood
glucose monitoring, insulin administration, IV meds (mostly IV antibiotics, Lasix and bumex), TPN, tube feedings,
wound care (including wound measurement), lab draws and specimen collection, foley/cath care, trach care, ADL
assistance, assessment charting, incident reports, monitoring vitals, oxygen administration/management, etc.
Nurses may assist with wound care - facilities try to keep wound care assigned to 1-2 consistent nurses, which
may include DNS.
- Experience: Need to have experience in smaller, rural facilities.
- Are there IVs or titratable drips: Antibiotics, hydration
- Are there Tele monitors: No telemetry
- DON/Charge Nurse/House Supervisor: DNS (DON) & Charge nurse (charge can be LPN or RN).
- Physician availability after hours: Differs per facility. Will contact the office during regular business hours or use
the on-call phone number as needed.
- Interdisciplinary Team: May include MDS coordinator, dietary, activities, and social services.
- Is RT on-site 24/7: No RT staff. O2, nebs, inhalers, established trachs, Cpap, Bipap.
- Pharmacy on-site: All facilities use local pharmacies with the exception of Fullerton (which uses Community
Pharmacy). All facilities use bubble packs with the exception of Franklin (cassettes).
- Shifts & Scheduling: 12Hr shifts, 6a-6p/6p-6a. Will consider block scheduling, dependent on facility - no
guarantees.
- Weekend Requirements: Every other weekend
- Holiday Requirements: Rotated
- Call or Standby Requirement: None
- Floating Requirements: Will work throughout the building. Rare occasion to float to another facility if picking
up PRN shifts.
- Scrub Color/Dress Code: Any color clean, professional scrubs.
- Unit/Department Specific Orientation: 1 day.
