Job description

The LPN Quality Care Coordinator serves our patients by working closely with our providers to manage patient care plans for our chronically ill patients. This position will primarily interact with patients over the phone to provide patient education, follow up on recent emergency room visits, and triage patients that have medical concerns. This position will also have some contact with patient in the clinic providing general nursing duties
Essential Functions:

  • Manages the care of patients with chronic conditions by assessing, planning, and coordinating all aspects of care in partnership with providers per Medical Director guidance.
  • Provides educates to patients about their conditions and treatments; assists patients with staying on tract with their treatment plan.
  • Follows up with patient regarding recent emergency department visits.
  • Oversees Transition Care Management services by setting up appropriate care for patients discharging from the hospital, as well as completing the required documentation.
  • Works closely with providers to clarify information and treatment plans and relay the information to the patient as needed.
  • Performs various medical procedures (within scope of practice) and general nursing duties such as taking blood pressure, respiration, IV therapy, catheterization, and observing symptoms.
  • Performs phone triage within the scope of practice for patients that have medical concerns.
  • Demonstrate the knowledge and skill necessary to provide care/service appropriate to the age of the patient.

Work Environment:
This position will work in an office environment with a moderate noise level, primarily interacting with patients over the phone and with some in-person interaction. This role will interact with patients, coworkers, venders, and the public. This role will use standard office equipment such as computers, phones, copiers, filing cabinets, and fax machines. Must be able to communicate in fluently in English when writing and speaking.

This position is primarily a sedentary role and will need to communicate over the phone, use standard office equipment, file, and other clerical duties. This position will also have in-person contact with patients and provide medical care including, but not limited to IV therapy, catherization, vaccinations, medication, and other general nursing duties. This position may come into contact with bodily fluids. This position may be required to lift up to 20-30 pounds.
Travel:
Minimal local travel to various clinics and locations in the company may be needed for this position.
Education and Experience:
  • Education:
    • Completion of an accredited LPN Program: Required
  • Experience:
    • 2+ years experience LPN: Required
    • 1+ years experience with IV Therapy and catheterization: Preferred
  • Certification:
    • Current Washington State LPN License: Required
    • Current ACLS or BLS: Preferred
