The Hospice LPN’s primary responsibility is to provide direct client care in accordance with physician’s orders working under the supervision of a Registered Nurse.
This role provides continuous care visits under the supervision and direction of the Hospice RN and in accordance with the plan of care. This includes on-call overnight shifts for continuous care.
Responsibilities:
- Provide direct patient care as assigned. Report all findings to the RN Case Manager, physician and Clinical Supervisor
- Update the nursing care plan as needed per physician orders. Document communication and interpret plan of care to the client regarding diagnosis and treatment
- Appropriately utilize and maintain nursing equipment and supplies needed for client care
- Corresponds with clinical field staff leaving messages and reports
- Participates in monthly rotation coverage schedule per policy
- Identify problems and potential problem situations to Clinical Director of Nursing as they become evident to RNCM, Clinical Supervisor and/or as they become evident
- Is computer literate, enters information into computer in a timely and accurate manner. Uses electronic means of communication effective within agency
- Graduate of an accredited school of nursing
- Currently licensed as a Licensed Practical Nurse in the State of Connecticut
- A minimum of one year of clinical experience
- Preferred, minimum one year of home care experience with a strong knowledge base of homecare and associated regulations
- Ability to handle several situations simultaneously in a professional, calm, dependable manner
