Position Overview: Centria Healthcare is seeking compassionate and skilled Home Care Nurses to join our team in Plymouth, Michigan and surrounding cities. As a Home Care Nurse, you will play a pivotal role in providing specialized nursing care to children and adults with complex medical needs within the comfort of their own homes. - 7:00am-3:00pm (8 hour shift)
- 8:00am-4:00pm (8 hour shift)
- 9:00am-5:00pm (8 hour shift)
- 10:00am-6:00pm (8 hour shift)
- 10:00pm-6:00pm (8 hour shift)
- 3:00pm-11:00pm (8 hour shift)
- 8:00am-6:00pm (10 hour shift)
- 10:00pm-8:00am (10 hour shift)
- 7:00am-7:00pm (12 hour shift)
- 11:00pm-7:00am (8 hour shift)
- 9:00am-9:00pm or 10:00am-10:00pm (12 hour shifts)

Nurse Centric Scheduling

Our nurses also work with our families to create personalized schedules in a way that works for all involved.
Responsibilities
- Perform duties outlined in patient's plan of care
- TBI + SCI rehabilitation
- Pediatrics
- Geriatrics
- Ventilator and Tracheotomy care (Training Provided)
- Gastrostomy tube care (Training Provided)
- Dispensing medications
- Observe client’s physical, mental and emotional conditions.
- Report any changes
- Document observed changes
- Communicates effectively with Clinical team to increase optimal outcomes
- Maintains professional nature within the home setting
Required Qualifications, Skills, and Knowledge
- Excellent oral and written communication skills
- Ability to follow verbal and written instructions
- Ability to utilize computers/tablets and computer software
- Current state license as an RN or LPN
- Valid driver’s license and reliable transportation
- Current negative TB skin test or negative chest X-ray
- Current flu vaccine
- CPR certified
