LPN
Duties:
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient vital signs and medical history
- Collaborate with interdisciplinary healthcare team members to provide comprehensive patient care
- Educate patients and their families on disease management, treatment plans, and preventive care measures
-Assist in wound care
- Perform health coaching to promote patient self-care and adherence to treatment plans
- Ensure infection control protocols are followed to maintain a safe healthcare environment
Skills:
- Current LPN license in [state]
- Strong clinical skills and knowledge of medical terminology
- Proficiency in electronic health record systems, such as eClinicalWorks and Athenahealth
- Ability to accurately code medical procedures and diagnoses for billing purposes
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to lift and move patients safely according to established protocols
