LPN - Licensed Practical Nurse- Part Time
JOB DUTIES SUMMARY:
Responsible for assuring integration of the various aspects of the resident’s program on 24/7 basis. Assists with coordinating, supervising and monitoring implementation of each resident’s service plan. Responsible for recommending modification to resident’s individual service plan to accommodate progress. Accompanies residents to medical, dental, psychiatric, and other related appointments. Required to transport residents. Required to work in any STARC operated Residential facility. Serves as backup to Registered Nurse.
ACCOUNTABILITY: Reports to residential Services Administrator.
WORK SCHEDULE: Minimum 15 hours, Monday - Friday with rotation of direct care and LPN duties.
ESSENTIAL FUNCTIONS:
- Completes monthly and quarterly medical assessments as assigned.
- Works assigned direct care shifts in accordance with weekly rotation schedules.
- Documents teaching methodology utilized in implementation of individual service plans.
- Recommends needed changes in resident’s individualized plans.
- Participates in and may facilitate interdisciplinary (I.D.) team meetings.
- Participates in and may conduct annual planning meetings.
- Participates in investigation related to injury and incident reports.
- Participates in development of behavior support plans and ensures adherence to plans when approved by ID team.
- May assist in scheduling of and maintaining minutes from routine community home meetings with staff and residents.
- Conducts assessments to determine resident’s strengths and training needs and makes recommendations to ID team regarding planned strategies and methodologies to be included in Individuals Service Plans (ICAPS)
- Assists with supervision of staff and coordination of medical aspects of program.
- Attends resident-specific meetings as needed.
- Meets with residents, families, educators, state, and regional officials and communicates with families as requested.
- Assists with ensuring that all residential services employees adhere to all mandated licensing requirements, guidelines, procedures and STARC policies.
- Accompanies residents to medical, dental, psychiatric, and other related appointments. Notes significant changes resultant of such appointments by memo to Residential Services Administrator, Staff RN, QIDP, Lead Staff or other appropriate staff.
- Coordinates with other programs/departments as needed to meet resident’s needs.

Heavy Work:
The employee must regularly lift and/or move 10 to 20 pounds, frequently lift, and/or move 25 to 50 pounds, and occasionally lift and/or move 50 to 100 pounds. While performing the duties of this job, the employee is regularly required to walk, stand, sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to bend over, stoop, kneel, or crouch. The employee is required to transfer individuals with physical disabilities to/from bathtubs, vehicles, beds, etc. The employee is also required to restrain adult individuals with intellectual, developmental, and physical disabilities when their behavior is aggressive.
OTHER RESPONSIBILITIES:
- Serves as back up to Registered Nurse in times of absence within scope of practice.
- Serves as public speaker and ambassador for STARC and the individuals STARC serves.
- Performs other duties as assigned.
EDUCATION, EXPERIENCE, and SKILLS REQUIRED:
- Active, unrestricted LPN license from Louisiana State Board of Nursing.
- Documentation of completed education program of Licensed Practical Nursing.
- Prefer minimum of one year of experience working directly with individuals with intellectual and developmental disABILITIES and one (1) or more years work experience.
- Current Driver's License
