LPN (Licensed Practical Nurse) Diabetic AM SHORT SHIFTS

Position:Licensed Practical Nurse

Location: PALMYRA

Schedule Available: Monday thru Friday 8a to 11a

Pay: $30/hr

Patient Care required:

• LPN to provide medication management and administration
• LPN to provide diabetic management
• LPN to provide CHF management/monitoring
• LPN to provide supervision and safety assurance
• LPN to complete full patient assessments
• LPN to communicate regularly with RN case manager and PCP as needed.

Experience: 1 year of Nursing preferably in the field of Home Health.

To be considered for this position, candidates must have the following:

LPN License

2 Forms of I.D
