Duties:
- Seeking an LPN with experience in geriatric long-term care as well as acute rehabilitation care.
- Participate in the assessment process by collecting information and reporting to the Registered Nurse.
- Assist the Registered Nurse in the formulation of the meaningful plan of care.
- Provides information on patient progress to RN in order to have input into plan of care revision.
- Monitor patient nutrition and fluid balance.
- Provides emotional support to the patient and family.

Salary: $49.00 - $50.00 per hour
